Commitment papers, 1851-1943.

Jacksonville patient commitment documents from county courts include jury or commission answers to interrogatory concerning patient's mental condition; jury verdict or commission case findings report; and warrant of commitment. Occasionally included are patient medical examinations; superintendent's patient receipt acknowledgement; and patient expense bonds. Information provided includes patient's name, age, sex, race, marital status, occupation, birthplace, town and county of residence; United States and Illinois residency length; parent information (e.g., names; birthplaces; death age; education; literacy); patient or family medical history; assigned insanity cause; current attack description; patient physical and mental condition at judicial inquiry; financial ability statement; juror or commissioner names; presiding judge; hearing date; filing date with clerk; and person transporting patient to the hospital. A small number of commitment papers (1872-1906) concern patients initially admitted to state hospitals at Anna, Elgin, Kankakee, Peoria, and Watertown who were subsequently transferred to Jacksonville.
